Why won't my avatar changes take effect, even after repeated saves. The old avatar remains unchanged.?

My avatar changes refuse to take effect, no matter how many times I save them. The old avatar remains unchanged. I know how to create/edit/save avatars, that's not the problem. I have the current flash player, that's not the problem. What's the fix here?
